SUMMARY INTRODUCTION: The breasts are symbols of femininity, sexuality, and maternity. Breast augmentation is among the most sought-after procedures for women and has a positive impact on quality of life. Sexuality is one of the items that contribute to increased quality of life. Surgical outcomes can be evaluated from the patients' perspective using developed and validated questionnaires. For the assessment of sexuality, the most commonly used instruments are the Female Sexual Quotient and the Female Sexual Function Index, which estimate several domains of sexuality and can be used to evaluate the impact of surgery on it. OBJECTIVE: The objective of this study was to evaluate the impact of breast augmentation on female sexuality. METHODS: We selected 87 patients from the Plastic Surgery Outpatient Clinic of Hospital São Paulo (Federal University of São Paulo) who wished to undergo breast augmentation. The patients were classified into two groups: the Female Sexual Quotient questionnaire was applied to one group, and the Female Sexual Function Index questionnaire was applied to the other group to evaluate sexuality preoperatively as well as at 2 and 4 months postoperatively. RESULTS: In both groups, there was a significant increase in the total score of the Female Sexual Quotient and Female Sexual Function Index questionnaires, and an individual increase in each domain assessed, with a significant increase in the domains of orgasm and sexual satisfaction, as well as foreplay and arousal, indicating an improvement in the patients' sexuality postoperatively. CONCLUSION: Breast augmentation has a positive impact on female sexuality; furthermore, the Female Sexual Quotient and Female Sexual Function Index are sensitive in detecting this impact.

SUMMARY OBJECTIVE: This study aimed to translate the BREAST-Q© — Breast Reconstruction Expectations Module (preoperative) 2.0 into Portuguese and adapt it to the Brazilian cultural context. METHODS: Authorization for translation and cross-cultural adaptation of the questionnaire was obtained from the holders of the instrument's distribution rights. The questionnaire was translated and retro-translated. For cultural adaptation, the instrument was applied to 40 patients who had breast reconstruction surgery scheduled. Cronbach's alpha was used to assess the internal consistency. RESULTS: The mean age of the patients was 53.5 years, and the majority (72.5%) was undergoing reconstruction with implants. Good and excellent internal consistencies were observed for the Coping and Appearance expectations scales (Cronbach's alpha values of 0.878 and 0.909, respectively). For the Pain scale, the internal consistency was moderate (0.738), and it was acceptable (0.587) for the Medical team. CONCLUSION: The BREAST-Q© — Breast Reconstruction Expectations Module (preoperative) 2.0 was successfully translated and adapted to the Brazilian context.

Introdução: As medidas precisas dos seios femininos são difíceis de obter devido à topografia, volume e projeções presentes. Portanto, este estudo teve como objetivo comparar medidas da região mamária obtidas por antropometria direta (fita métrica) e indireta (fotogrametria computadorizada). Métodos: Este é um estudo transversal. Foram avaliadas 40 mulheres, com idades entre 18 e 60 anos, índice de massa corporal <29,2 kg / m², que tinham 12 marcas anatômicas na região das mamas e braços. A união desses pontos formou 7 segmentos lineares e 1 ângulo para cada hemicorpo, e 1 segmento comum a ambos os hemicorpos. As fotografias obtidas de forma padronizada foram mensuradas por fotogrametria computadorizada com o software Image Tool®. Os mesmos segmentos também foram medidos por antropometria direta, com fita métrica. O teste de Shapiro-Wilk foi usado para avaliar se cada variável tinha uma distribuição normal. O teste de correlação de Pearson foi aplicado para avaliar a correlação entre os diferentes métodos: antropometria direta (medição com fita) e indireta (fotogrametria por Image Tool®). O nível de significância adotado para os testes estatísticos foi de 5% (p <0,05). Resultados: Diferenças significativas foram encontradas nas comparações entre a fita métrica e a fotogrametria computadorizada para todos os segmentos analisados (p> 0,05). Conclusão: Existe correlação entre as medidas dos seios da face obtidas pela antropometria direta (medição com fita métrica) e indireta (fotogrametria pelo software Image Tool®), principalmente nas medidas da papila.


Introduction: Accurate female breast measurements are difficult due to the topography, volume, and projections present there. Therefore, this study aimed to compare breast region measurements obtained by direct (tape measurement) and indirect (computer-based photogrammetry) anthropometry. Methods: This is a transversal study. Forty women were evaluated, aged 18-60 years, body mass index of <29.2kg/m² that had 12 anatomical marks on the breast region and arms. These points' union formed 7 linear segments and 1 angle for each hemibody, and 1 segment common to both hemibodies. The photographs obtained in a standardized way were measured using computer-based photogrammetry with Image ToolTM software. The same segments were also measured by direct anthropometry, using a tape measure. The Shapiro-Wilk test was used to assess whether each variable was normally distributed. The Pearson correlation test was applied to evaluate the correlation between different methods: the direct (tape measurement) and indirect (photogrammetry by Image ToolTM) anthropometry. The significance level adopted for statistical tests was 5% (p<0,05). Results: Significant differences were found in the comparisons between the tape measurement and computer-based photogrammetry for all segments analyzed (p>0.05). Conclusion: There is a correlation between the breast measurements obtained by direct (tape measurement) and indirect (photogrammetry by Image ToolTM software) anthropometry, especially the papilla measures.

Abstract Purpose: To compare JPEG and RAW image file extensions to direct measurement of the breast region. Methods: Points were marked on the breasts and arms of 40 female volunteers. The joining of these points in each hemibody formed seven linear segments, one angular segment and one median segment common to both hemibodies. Volunteers were photographed in a standardized fashion and evaluated by three raters using the software Adobe Photoshop CS6® and three image file extensions (RAW, high resolution JPEG and low resolution JPEG); values were compared to direct anthropometry. Results: All variables had interclass correlation coefficient higher than 0.8 (ICC>0.8). On average, all variables in all methods showed differences (p<0.05) when compared to direct measurement. A formula was created for each segment and each image file extension in comparison with the direct measurement. Conclusion: Measurements were similar among the correlated JPEG and RAW image file extensions but differed from the actual breast measurement obtained with a caliper.

Introdução: Em dois estudos prévios, avaliou-se a qualidade dos ensaios clínicos aleatórios (ECAs) com a participação de pelo menos um cirurgião plástico, em dois períodos: 1966 a 2003 e 2004 a 2008. O objetivo é avaliar a evolução da qualidade das publicações de ECAs por cirurgiões plásticos no período subsequente de cinco anos, de 2009 a 2013. Métodos: ECAs publicados de 2009 a 2013, em língua inglesa, com a participação de pelo menos um cirurgião plástico, foram identificados por busca eletrônica e classificados quanto ao sigilo de alocação, por dois avaliadores independentes. Os estudos com sigilo de alocação adequado tiveram a qualidade avaliada por dois avaliadores, utilizando-se a Lista de Delphi e a Escala de Qualidade de Jadad. Resultados: Dos 6.997 estudos identificados, 261 foram classificados quanto ao sigilo de alocação. Destes, 43 (16,47%) tinham sigilo de alocação adequado. Segundo a avaliação pela Lista de Delphi, houve melhora, em relação a 1966-2003, nos itens "características mais importantes do prognóstico" (p < 0,001), "uso de avaliador independente" (p = 0,0029) e "medidas de variabilidade e estimativa de pontos para a variável primária" (p = 0,0057); não houve diferença em relação a 2004-2008. Quanto à Escala de Qualidade de Jadad, houve um aumento dos escores em relação a 1996-2003 (p < 0,0004), mas também sem diferença significante em relação ao período 2004-2008. Conclusão: Não houve diferença na qualidade das publicações de ECAs por cirurgiões plásticos no período de 2009 a 2013, em relação aos cinco anos anteriores (2004 a 2008). Entretanto, ambos os períodos apresentaram maior qualidade quando comparados ao período de 1966 a 2003.


Introduction: In two previous studies, the quality of randomized clinical trials (RCTs) with the participation of at least one plastic surgeon was assessed in two periods: from 1966 to 2003 and from 2004 to 2008. The objective is to evaluate the evolution of the quality of RCTs published by plastic surgeons in the subsequent five-year period, from 2009 to 2013. Methods: RCTs published from 2009 to 2013, in English, with the participation of at least one plastic surgeon, were identified by an electronic search and classified according to allocation concealment by two independent evaluators. The quality of the studies with adequate allocation concealment was evaluated by two evaluators using the Delphi List and the Jadad Scale. Results: Of the 6,997 identified studies, 261 were classified according to allocation concealment. Of these, 43 (16.47%) had adequate allocation concealment. According to an assessment conducted using the Delphi List, there was an improvement in the items "most important characteristics of the prognosis" (p < 0.001), "use of an independent evaluator" (p = 0.0029), and "measures of variability and estimation of points for the primary variable" (p = 0.0057) compared to the 1966-2003 assessment ; there was no difference in the assessment of the same items from 2004-2008. Regarding the Jadad Scale, there was an increase in the scores from 2009 to 2013 compared to the 1996-2003 period (p < 0.0004); however, there was no significant difference in the 2004-2008 period. Conclusion: There was no difference in the quality of the RCTs published by plastic surgeons in the 2009-2013 period compared to the previous five-year period (2004 to 2008). However, both periods indicated higher quality compared to the 1966-2003 period.

OBJECTIVES: To evaluate the impact of breast reconstruction after mastectomy on sexual function, body image, and depression. METHODS: This cross-sectional, comparative, controlled study was conducted with 90 women between 18 and 65 years of age who had undergone either mastectomy alone (mastectomy group, n=30) or mastectomy combined with breast reconstruction (mastectomy-reconstruction group, n=30) at least one year prior to the study or who had no breast cancer (control group, n=30). Patients were assessed for sexual function, depression, and body image using the validated Brazilian-Portuguese versions of the Female Sexual Function Index, the Beck Depression Inventory, and the Body Dysmorphic Disorder Examination, respectively. RESULTS: The three groups were homogeneous for age, marital status, body mass index, and education level. The women in the mastectomy group reported significantly worse sexual function, greater depressive symptoms, and lower body image than those in the mastectomy-reconstruction and control groups. In the mastectomy group, the frequency of sexual dysfunction was significantly greater among patients without a marital partner and those with a higher level of education than among patients in the other two groups with the same characteristics. CONCLUSION: Patients who had undergone breast reconstruction after mastectomy reported better sexual function and body image and fewer depressive symptoms than patients who had undergone mastectomy alone. Sexual dysfunction was associated with the absence of a marital partner and a higher level of education and was more frequent in the mastectomy group.

ABSTRACT BACKGROUND: Breast cancer is the second most frequent type of cancer worldwide and the most common type among women. The treatment for this condition has evolved over recent decades with therapeutic and technological advances. Breast reconstruction techniques using musculocutaneous flaps from the latissimus dorsi and rectus abdominis have aroused interest regarding patients' quality of life. Our goal here was to compare patients' satisfaction scores after they underwent breast reconstruction using musculocutaneous flaps from either the latissimus dorsi or the rectus abdominis. DESIGN AND SETTING: Primary, clinical, analytical, observational and cross-sectional study conducted in a federal university and a public hospital. METHODS: Demographic and clinical data were collected. The Mini-Mental State Examination was then applied, with testing for specificity and sensitivity. Lastly, a breast evaluation questionnaire was applied to evaluate breast satisfaction among 90 women, who were divided into three groups: mastectomy (control; n = 30); breast reconstruction using flap from the latissimus dorsi (n = 30); and reconstruction using flap from the rectus abdominis (n = 30). RESULTS: The groups were homogeneous regarding the main demographic data and the questionnaire responses (P < 0.05). Compared with the control group, the reconstruction groups showed significant improvement in satisfaction (P < 0.0002) after one year. CONCLUSION: Within our sample, women who underwent breast reconstruction with flaps from either the latissimus dorsi or the rectus abdominis had similar satisfaction scores.

Introdução: Com a globalização e melhora dos meios digitais e, consequentemente, a crescente utilização de fotografias digitais, faz-se necessária a criação de métodos de mensuração confiáveis. Na prática clínica, a medida direta utilizando-se fita métrica, régua e compasso é mais comumente utilizada. Porém, existem fatores intrínsecos (relevos corporais) e extrínsecos (colaboração do paciente ou movimentos respiratórios) que podem influenciar nos dados obtidos. A fotografia e os softwares gráficos podem ser uma alternativa precisa, mais cômoda e reprodutível à medida direta. O objetivo é demonstrar a forma de medir as mamas pelo software Adobe Photoshop. Métodos: Foram descritas a posição das voluntárias, a captação das imagens, calibração do software até a obtenção mensuração final das mamas. Em 40 mulheres voluntárias, com idade entre 18 e 60 anos, foram demarcados pontos sobre a região mamária e braços em posição frontal. A união destes pontos em cada hemicorpo formou 7 segmentos lineares, 1 angular e 1 segmento mediano comum a ambos os hemicorpos. Essas fotografias foram avaliadas no arquivo RAW, por 3 observadores, com auxílio do software Adobe Photoshop CS6®. Resultados: O Photoshop CS6® demonstrou ser eficaz na mensuração das mamas no arquivo RAW. Conclusão: O Photoshop® apresentou precisão nas medidas e ser factível para mensuração das mamas.


Introduction: With the globalization and improvement of digital media and, consequently, the increasing use of digital photography, it is necessary to create reliable measurement methods. In clinical practice, direct measurement using tape measure, ruler, and compass, is most commonly used. However, there are intrinsic (body contour) and extrinsic (patient collaboration or respiratory movements) factors that may influence the data obtained. Photography and graphics software can be an accurate, comfortable, and reproducible alternative to direct measurement. The objective is to demonstrate how to perform breast measurement using Adobe Photoshop software. Methods: Participants were described the position to assume. Imaging capturing techniques and software calibration were performed in order to obtain the final breast measurement. Markers were placed on the breast and arms of 40 women volunteers aged 18 to 60 years using an anterior view. The union of these markers in each hemibody resulted in seven linear segments, one angle and one transverse segment common to both hemibodies. These photographs were evaluated as a RAW file, by 3 evaluators, assisted by Adobe Photoshop CS6® software. Results: Photoshop CS6® proved effective in the breast measurement using RAW files. Conclusion: Photoshop® showed precision and effectiveness for breast measurement.

Introdução: Mensurações por antropometria indireta são importantes na área de pesquisa, mas também podem se tornar significativas na área jurídica, como instrumento quantitativo de avaliação pré e pós-operatória. O tipo de arquivo utilizado determina variações na manipulação das imagens. O objetivo é evidenciar tipos de arquivos de imagens utilizados na fotografia digital que será utilizada na pesquisa. Métodos: Foram realizadas mensurações das mamas em 80 hemitórax de 40 mulheres voluntárias. Foram demarcados pontos sobre a região mamária e braços em posição frontal. A união destes pontos em cada hemicorpo formou sete segmentos lineares, uma medida angular e um segmento mediano comum a ambos os hemicorpos. Essas fotografias foram avaliadas em arquivos RAW e JPEG de alta definição, por 3 observadores, com auxílio do software Adobe Photoshop CS6®. Resultados: Os arquivos RAW e JPEG demonstraram serem eficazes na mensuração das mamas. Conclusão: Os arquivos RAW e JPEG apresentaram medidas semelhantes na mensuração das mamas femininas.


Introduction: Indirect anthropometric measurements are important in research but may also be significant in the legal area as a quantitative instrument for pre- and post-operative evaluation. The type of file used determines variations in the manipulation of the images. The objective is to study the types of image files used in digital photography that will find utility in research. Methods: The breasts of 40 women volunteers were measured in 80 hemithoraces, and the mammary region and arms were marked in the frontal position. The union of these points in each hemithorax formed seven linear segments, an angular measure, and a median segment common to both hemithoraces. These photographs were evaluated as high definition RAW and JPEG files by three observers using Adobe Photoshop CS6® software. Results: RAW and JPEG files were shown to be effective in breast measurement. Conclusion: RAW and JPEG files presented similar results in the measurement of female breasts.

OBJECTIVE: The aim of this study was to analyse patient-reported outcomes of reduction mammoplasty among Brazilian women. METHODS: A total of 100 women were enrolled in this cross-sectional controlled study, 50 with breast hypertrophy (Hypertrophy Group) and 50 who had undergone breast reduction at least six and up to 12 months before (Mammoplasty Group). The Brazilian version of the BREAST-Q® was applied to assess patient-reported outcomes. The module reduction/mastopexy was used, and the preoperative and postoperative versions were applied to the hypertrophy and mammoplasty groups, respectively. RESULTS: The mammoplasty group presented higher scores for the subscales satisfaction with breasts, psychosocial well-being, sexual well-being and physical well-being (p=0.0001 for all of these subscales). CONCLUSION: These results suggest that patients submitted to reduction mammoplasty are satisfied with the outcomes and present better quality of life scores compared with women with breast hypertrophy.

Introdução: Apesar do avanço no diagnóstico e no tratamento das pacientes com neoplasia mamária, ainda há casos que se apresentam como doença localmente avançada. Nesse contexto, o controle oncológico requer ressecções extensas e complexos procedimentos reparadores. A participação do cirurgião plástico nas equipes multidisciplinares de tratamento oncológico tem apresentado importância crescente tanto nas etapas de planejamento como de execução. O objetivo é analisar prospectivamente casos de pacientes com extensas ressecções no tórax e reconstrução de parede torácica quanto aos seus resultados e complicações no período de 2014-2016. Métodos: Seguimento prospectivo por 6 meses de 15 casos de pacientes submetidas a extensas ressecções oncológicas no tórax e reconstrução por um mesmo cirurgião plástico em hospitais da rede pública de São Paulo-SP. Foram coletados os dados: idade, diagnóstico, queixa pré-operatória, extensão da área de ressecção e incidência de complicações locais. Resultados: As pacientes tinham idade média de 49,2 anos, a queixa mais comum era massa papável, o diagnóstico mais comum era neoplasia da mama (80%), a maioria das ressecções preservou o plano muscular e a área média de ressecção foi de 259,2 cm2. O retalho miocutâneo do grande dorsal em V-Y foi o mais utilizado, e o toracoabdominal foi o segundo. Epiteliólise foi detectada em 13,3% das pacientes e necrose parcial menor que 5% do retalho em 13,3%. Conclusão: Na presente casuística, as pacientes portadoras de neoplasias extensas no tórax representaram um desafio para a reconstrução local, necessitando de planejamento pré-operatório minucioso e com múltiplas opções. A participação do cirurgião plástico no tratamento desses casos contribuiu para redução de morbidade e demonstrou baixo índice de complicações.


Introduction: In spite of advances in the diagnosis and treatment of patients with breast cancer, there are still cases that present with locally advanced disease. In this context, cancer control requires extensive resections and complex repair procedures. The participation of the plastic surgeon in the multidisciplinary cancer treatment teams has been increasingly important in both the planning and execution stages. The aim of this study is to prospectively analyze patients who underwent extensive chest resections and thoracic wall reconstruction with regard to their results and complications in the period 2014-2016. Methods: Prospective 6-month follow-up of 15 patients who underwent extensive oncological resections in the chest and reconstruction by the same plastic surgeon in public hospitals of São Paulo was done. The following data were collected: age, diagnosis, preoperative complaint, extent of resection area, and incidence of local complications. Results: Patients had a mean age of 49.2 years, the most common complaint was a papillary mass, the most common diagnosis was breast cancer (80%), the most resection area preserved was the muscular plane, and the mean resection area was 259.2 cm2. The V-Y latissimus dorsi myocutaneous flap was the most used, and the thoracoabdominal flap was the second. Epitheliolysis was detected in 13.3% of the patients, and partial necrosis less than 5% of the flap in 13.3%. Conclusion: In the present study, patients with extensive cancers in the thorax presented a challenge for local reconstruction, requiring detailed preoperative planning and multiple options. The participation of the plastic surgeon in the treatment of these patients contributed to the reduction of morbidity rate and low rate of complications.

ABSTRACT CONTEXT AND OBJECTIVE: Patient-reported outcome measurements assessing the emotional state of children and adolescents who seek plastic surgery are important for determining whether the intervention is indicated or not. The aim of this study was to cross-culturally adapt and validate the Short Mood and Feelings Questionnaire (child/adolescent and parent versions) for Brazilian Portuguese, test its psychometric properties and assess the emotional state of children and adolescents who seek plastic surgery. DESIGN AND SETTING: Cross-cultural validation study conducted in a plastic surgery outpatient clinic at a public university hospital. METHODS: A total of 124 consecutive patients of both sexes were selected between September 2013 and February 2014. Forty-seven patients participated in the cultural adaptation of the questionnaire. The final version was tested for reliability on 20 patients. Construct validity was tested on 57 patients by correlating the Short Mood and Feelings Questionnaire (child/adolescent and parent versions) with the Strengths and Difficulties Questionnaire and the Rosenberg Self-Esteem scale. RESULTS: The child/adolescent and parent versions of the Short Mood and Feelings Questionnaire showed Cronbach's alpha of 0.768 and 0.874, respectively, and had good inter-rater reliability (intraclass correlation coefficient, ICC = 0.757 and ICC = 0.853, respectively) and intra-rater reliability (ICC = 0.738 and ICC = 0.796, respectively). CONCLUSIONS: The Brazilian-Portuguese version of the Short Mood and Feelings Questionnaire is a reproducible instrument with face, content and construct validity.The mood state and feelings among children and adolescents seeking cosmetic surgery were healthy.

Introdução: Após o diagnóstico e tratamento do câncer de mama, algumas mulheres passam por alterações físicas, sociais e emocionais que repercutem muitas vezes na qualidade de vida e na autoestima. O objetivo é avaliar a qualidade de vida e autoestima de pacientes mastectomizadas submetidas ou não à reconstrução mamária após um mês de cirurgia. Métodos: A casuística foi composta por 89 pacientes, com idade igual ou superior a 30 anos, submetidas ou não à reconstrução mamária, e oriundas dos ambulatórios de Cirurgia Plástica e Mastologia do Hospital São Paulo e do Hospital Pérola Byington. O grupo 1 (n = 30) composto pelas pacientes mastectomizadas sem reconstrução mamária; o grupo 2 (n = 29) mastectomizadas com reconstrução mamária; e o grupo 3 (n = 30) pacientes sem alterações nas mamas. Para a avaliação da qualidade de vida, foram aplicados os instrumentos validados para uso no Brasil EORTC QLQ-C30 e Escala de Autoestima de Rosenberg UNIFESP/EPM. Resultados: Não foram observadas repercussões na qualidade de vida e autoestima das pacientes submetidas à mastectomia com ou sem a reconstrução mamária após um mês de cirurgia. Conclusão: Mulheres mastectomizadas com ou sem reconstrução mamária após um mês da cirurgia não apresentaram repercussões na qualidade de vida e na autoestima, quando comparadas entre si, bem como comparadas às mulheres sem história de câncer. É necessário que este tipo de avaliação, por meio dos questionários EORTC QLQ-C30 e QLQ-BR23, seja realizado no momento do diagnóstico do câncer de mama até o momento da pós-reconstrução mamária, com a finalidade de identificar em que fase se instalam as repercussões na qualidade de vida e autoestima.


Introduction: After diagnosis and treatment of breast cancer, some women go through physical, social, and emotional changes that often have an impact on their quality of life and self-esteem. This study aimed to assess the quality of life and self-esteem of mastectomized patients with or without breast reconstruction a month after surgery. Methods: The study consisted of 89 patients aged ≥ 30 years with or without breast reconstruction. They were recruited from the Plastic Surgery and Mastology clinic of São Paulo Hospital and Pérola Byington Hospital. Group 1 (n = 30) consisted of mastectomized patients without breast reconstruction; group 2 (n = 29) consisted of mastectomized patients with breast reconstruction; and group 3 (n = 30) consisted of patients without changes in the breasts. The EORTC QLQ-C30 and Rosenberg Self-Esteem Scale UNIFESP/EPM instruments, which are validated in Brazil, were used to assess their quality of life. Results: There were no observed effects in the quality of life and self-esteem of the patients who underwent mastectomy with or without breast reconstruction a month after surgery. Conclusion: The women with mastectomy with or without breast reconstruction a month after surgery showed no changes in their quality of life and self-esteem when compared among themselves and to women with no cancer history. It is necessary that this assessment, employing the EORTC QLQ-C30 and QLQ-BR23 questionnaires, be performed from the time of breast cancer diagnosis until after breast reconstruction to identify the stage when the quality of life and self-esteem are affected.

ABSTRACT

A Hiperplasia Estromal Pseudoangiomatosa (PASH) é uma doença benigna caracterizada pela proliferação excessiva de fibroblastos e miofibroblastos, podendo levar a um crescimento mamário importante. A apresentação é rara, em especial ocasionando necessidade de mastectomia em pacientes jovens. O estudo apresentou o relato de caso raro de uma paciente de 11 anos de idade, com hipertrofia mamária de rápida progressão, com necessidade de mastectomia e posteriormente mamoplastia de aumento para completa reinserção social.


Pseudoangiomatous stromal hyperplasia is a benign disease characterized by excessive proliferation of fibroblasts and myofibroblasts, which can lead to significant breast growth. The presentation is rare, especially among young women and cases requiring mastectomy. This report describes a rare case of an 11-year-old female patient with rapidly progressing mammary hypertrophy, who needed mastectomy and then mammoplasty for complete social integration.

ABSTRACT: CONTEXT AND OBJECTIVE: Rhinoplasty is one of the most sought-after esthetic operations among individuals with body dysmorphic disorder. The aim of this study was to cross-culturally adapt and validate the Body Dysmorphic Symptoms Scale. DESIGN AND SETTING: Cross-cultural validation study conducted in a plastic surgery outpatient clinic of a public university hospital. METHODS: Between February 2014 and March 2015, 80 consecutive patients of both sexes seeking rhinoplasty were selected. Thirty of them participated in the phase of cultural adaptation of the instrument. Reproducibility was tested on 20 patients and construct validity was assessed on 50 patients, with correlation against the Yale-Brown Obsessive Compulsive Scale for Body Dysmorphic Disorder. RESULTS: The Brazilian version of the instrument showed Cronbach's alpha of 0.805 and excellent inter-rater reproducibility (intraclass correlation coefficient, ICC = 0.873; P < 0.001) and intra-rater reproducibility (ICC = 0.939; P < 0.001). Significant differences in total scores were found between patients with and without symptoms (P < 0.001). A strong correlation (r = 0.841; P < 0.001) was observed between the Yale-Brown Obsessive Compulsive Scale for Body Dysmorphic Disorder and the Body Dysmorphic Symptoms Scale. The area under the receiver operating characteristic curve was 0.981, thus showing good accuracy for discriminating between presence and absence of symptoms of body dysmorphic disorder. Forty-six percent of the patients had body dysmorphic symptoms and 54% had moderate to severe appearance-related obsessive-compulsive symptoms. CONCLUSIONS: The Brazilian version of the Body Dysmorphic Symptoms Scale is a reproducible instrument that presents face, content and construct validity.


RESUMO CONTEXTO E OBJETIVO: Rinoplastia é uma das operações mais procuradas por indivíduos com o transtorno dismórfico corporal. O objetivo deste estudo foi adaptar culturalmente e validar a Body Dysmorphic Symptoms Scale. DESENHO E LOCAL: Estudo de validação cultural desenvolvido no ambulatório de cirurgia plástica de um hospital universitário público. MÉTODOS: Oitenta pacientes consecutivos de ambos os gêneros que desejavam submeter-se à rinoplastia foram selecionados entre fevereiro de 2014 e março de 2015. Trinta pacientes participaram da fase de adaptação cultural do instrumento. A reprodutibilidade foi testada em 20 pacientes e a validade de construto em 50 pacientes, correlacionando-se a escala com a Yale-Brown Obsessive Compulsive Scale para transtorno dismórfico corporal. RESULTADOS: A versão brasileira do instrumento mostrou alfa de Cronbach de 0,805 e excelente reprodutibilidade interobservador (coeficiente de correlação intraclasse, CCI = 0,873; P < 0,001) e intraobservador (CCI = 0,939; P < 0,001). Houve diferença significante entre os escores totais de pacientes com e sem sintomas (P < 0,001). Observou-se forte correlação (r = 0,841; P < 0,001) entre a Yale-Brown Obsessive Compulsive Scale para transtorno dismórfico corporal e a Body Dysmorphic Symptoms Scale. A área sob a curva característica de operação do receptor (ROC) foi de 0,981, revelando boa acurácia para discriminar a presença de sintomas para transtorno dismórfico corporal; 46% dos pacientes apresentaram sintomas do transtorno dismórfico corporal e 54% dos pacientes apresentaram sintomas obsessivo-compulsivos moderados a graves relacionados com a aparência. CONCLUSÃO: A versão brasileira da Body Dysmorphic Symptoms Scale é um instrumento reprodutível que apresenta validade de face, conteúdo e construto.

Introdução: A mama é símbolo de feminilidade, maternidade e sexualidade. Mamoplastia de aumento é a segunda cirurgia estética mais realizada no Brasil, e está relacionada à melhora da imagem corporal, autoestima e qualidade de vida. Sexualidade é importante componente da qualidade de vida. O quociente sexual feminino (QS-F) é questionário desenvolvido e validado que avalia sexualidade, aplicado para avaliar o impacto do tratamento na saúde sexual. Consiste em 10 questões, que avaliam quatro domínios: desejo, excitação, conforto e satisfação sexual. O objetivo é avaliar a sexualidade após mamoplastia de aumento. Métodos: 47 candidatas a aumento das mamas foram selecionadas, tratadas cirurgicamente, e responderam ao questionário QS-F no pré e no pós-operatório de 2, 4 e 18 meses. A técnica adotada foi semelhante para todas pacientes: incisão no sulco mamário, loja subglandular, implante redondo entre 260 e 325 mL. Foram formados os grupos: com ou sem relacionamento estável; e com ou sem estrias. Resultados: 45 pacientes concluíram o estudo, com idade média de 26 anos. As complicações observadas foram: simastia (1), galactorreia (1), e estrias (10). Houve melhora significante entre o escore total do QS-F no pré e no pós-operatório de 4 (p = 0,001) e 18 meses (p = 0,001), relacionado ao significante aumento da pontuação dos domínios excitação (p < 0,001) e satisfação sexual (p = 0,001). Nas pacientes que apresentaram estrias não houve melhora significante da sexualidade (p = 0,627), ao contrário das que não apresentaram (p < 0,001). Não houve diferença significante na sexualidade entre o grupo com ou sem relacionamento estável. Conclusão: A sexualidade aumentou significantemente após mamoplastia de aumento.


Introduction: The breasts are symbol of femininity, motherhood and sexuality. Breast augmentation is the second most performed cosmetic surgery in Brazil, and it is related to improvement of body image, self-esteem and quality of life. Sexuality is a significant component of quality of life. The female sexual quotient questionnaire (FS-Q) was validated as an evaluation tool for sexuality, and it is used to evaluate the impact of many treatments for sexual health. The questionnaire entails 10 questions, which assess four domains of sexuality: sexual desire, sexual arousal, comfort and sexual satisfaction. To evaluate sexuality after breast augmentation. Methods: We selected 47 patients with interest in breast enlargement. During treatment patients completed the FSQ questionnaire before and after 2, 4 and 18 months of the surgery. The surgical technique used was the same for all patients: inframammary incision, subglandular pocket, round implant, volume 260 to 325 mL. There were four groups: with or without stable relationship, and with or without striae. Results: A total of 45 patients were considered for the study. Patients' mean age was 26 years. Complications observed were: symmastia (1), galactorrhea (1), and striae distensae(10). Significant improvement was observed between the total FSQ score at preoperative, 4 months (p = 0.001), and postoperative, 18 months (p = 0.001), and also related with the significant increase in arousal scores (p < 0.001), and sexual satisfaction (p = 0.001). Patients with striae did not show significant improvement in sexuality (p=0.627), differently of patients without striae (p < 0.001). No significant difference in sexuality was seen between the group with or without stable relationship. Conclusion: Sexuality increases significantly after breast augmentation.

Objective: To translate, culturally adapt, and validate a Brazilian Portuguese version of the Yale-Brown Obsessive Compulsive Scale modified for Body Dysmorphic Disorder (BDD-YBOCS). Methods: Ninety-three patients of both sexes seeking rhinoplasty were consecutively selected at the Plastic Surgery Outpatient Clinic of the Universidade Federal de São Paulo, Brazil, between May 2012 and March 2013. The BDD-YBOCS was translated into Brazilian Portuguese. Thirty patients participated in the cultural adaptation of the scale. The final version was tested for reliability in 20 patients, and for construct validity in 43 patients (correlation of the BDD-YBOCS with the Body Dysmorphic Disorder Examination [BDDE]). Results: Total Cronbach’s alpha was 0.918. The BDD-YBOCS had excellent inter-rater (intra-class correlation coefficient [ICC] = 0.934; p < 0.001) and intra-rater reliability (ICC = 0.999; p < 0.001). Significant differences in BDD-YBOCS scores were found between patients with and without BDD symptoms (p < 0.001), and among patients with different levels of BDD severity (p < 0.001). A strong correlation (r = 0.781; p < 0.001) was observed between the BDDE and the BDD-YBOCS. The area under the receiver operating characteristic curve was 0.851, suggesting a very good accuracy for discriminating between presence and absence of BDD symptoms. Conclusion: The Brazilian Portuguese version of the BDD-YBOCS is a reliable instrument, showing face, content and construct validity.

ABSTRACT

PURPOSE: To determine the limitations of the photographs used to obtain the anthropometric measurements of the breast region. METHODS: Five women, between the ages of 18 to 60 years, were evaluated. Photographs of the frontal and left and right profile views of their breasts were taken. Based on the current literature, the most commonly used anthropometric and anatomic landmarks for breast measurement were marked in their different positions. The different points were used to evaluate if the direct anthropometry was possible in a standardized way and determine how the points and the positions can to be used in any breast measurements. RESULTS: There were some limitations to the use of defining points of the breast fold, as well as of its lower portion and lateral extension positions in both profiles. CONCLUSION: The defining points of the breast fold and the profile photographs have some limitations and we suggested how the points and positions can be used for breasts measurements. .

ABSTRACT

Objective: To show the way to reach and stay in note 6 in the evaluation process of Medicine III of CAPES. Methods: Capes determinations were reviewed concerning this topic, grades 6 and 7, and also the difficulties and facilities of running a program that amounted to Note 6 after restructuring and being in compliance with regulations. Results: The main points to achieve and maintain Note 6 were: 1) regular production of master's and doctoral theses with appropriate distribution among all teachers; 2) average time of appropriate titration, as well as strict selection of students who resets the withdrawals and cancellations; 3) production of scientific articles in high impact journals and with academic and student participation in most part; 4) progressive and substantial increase in fundraising and patent search; 5) progressive increase in international exchanges with joint production; 6) visibility through new bilingual website and updated weekly; 7) numerous solidarity activities in research, but also in health services for the population and even in basic education; 8) rigorous selection of students (through design analysis, curriculum and teacher training program); 9) maintenance of high levels teachers production; 10) preparing new teachers for guidance through participation as co-supervision and involvement in the program to fit the needs. Conclusion: The Postgraduate Program in Translational Surgery went through difficult times; was submitted to a series of measures, adjustments, cooperation and understanding of the teaching staff, that took the program from note 3 - and almost closing - to a level of excellence keeping note 6 for three consecutive three-year periods of evaluation.


Objetivo: Mostrar o caminho para alcançar e se manter na nota 6 no processo de avaliação da Medicina III da Capes. Métodos: Foram revisadas as determinações da Capes concernentes ao tema, conceitos 6 e 7, e também as dificuldades e facilidades próprias da execução de um programa que ascendeu à nota 6 após reestruturação e adequação às normas vigentes. Resultados: Os pontos principais para alcançar e manter conceito 6 foram: : 1) produção regular de teses de mestrado e doutorado com distribuição adequada entre todos os orientadores; 2) tempo médio de titulação adequado, assim como rígida seleção do alunado que zera as desistências e cancelamentos; 3) produção de artigos científicos alta em revistas de impacto e com participação docente e discente na sua grande maioria; 4) aumento progressivo e substancial da captação de recursos e busca de patentes; 5) aumento progressivo dos intercâmbios internacionais com produção conjunta; 6) visibilidade através de novo site bilíngue e atualizado semanalmente; 7) inúmeras atividades de solidariedade em pesquisa, mas também em Serviços de Saúde para a população e mesmo no ensino básico; 8) rigorosa seleção dos alunos (através de análise de projeto, currículo e programa de estágio docente); 9) manutenção de índices elevados de produção dos docentes; 10) preparação de novos docentes para orientação através de participação como co-orientação e envolvimento no programa para se adequar às necessidades. Conclusão: O Programa de Pós-Graduação em Cirurgia Translacional passou por momentos difíceis e que através de uma série de medidas, ajustes e a colaboração e compreensão do seu corpo docente pode sair da nota 3 e quase fechamento, para um nível de excelência mantendo nota 6 em três triênios seguidos.

PURPOSE: To compare breast measurements performed using the software packages ImageTool(r), AutoCAD(r) and Adobe Photoshop(r) with direct anthropometric measurements. METHODS: Points were marked on the breasts and arms of 40 volunteer women aged between 18 and 60 years. When connecting the points, seven linear segments and one angular measurement on each half of the body, and one medial segment common to both body halves were defined. The volunteers were photographed in a standardized manner. Photogrammetric measurements were performed by three independent observers using the three software packages and compared to direct anthropometric measurements made with calipers and a protractor. RESULTS: Measurements obtained with AutoCAD(r) were the most reproducible and those made with ImageTool(r) were the most similar to direct anthropometry, while measurements with Adobe Photoshop(r) showed the largest differences. Except for angular measurements, significant differences were found between measurements of line segments made using the three software packages and those obtained by direct anthropometry. CONCLUSION: AutoCAD(r) provided the highest precision and intermediate accuracy; ImageTool(r) had the highest accuracy and lowest precision; and Adobe Photoshop(r) showed intermediate precision and the worst accuracy among the three software packages. .
